Job Title: Community Support Specialist

Job Overview

We are actively seeking a dedicated Community Support Specialist to join our team at Compass Health Network. This full-time position is instrumental in addressing the critical service needs of the communities we serve. You will play a vital role in providing compassionate care and support, helping clients on their journey toward recovery and resilience.

Key Responsibilities

  • Deliver individualized and group educational services tailored to the strengths, needs, and preferences of clients and their families.
  • Collaborate with clients to create and develop Wellness Plans, facilitating their movement toward identified goals.
  • Offer holistic support by focusing on the Eight Dimensions of Wellness, ensuring a comprehensive approach to client care.
  • Assist clients in accessing public services such as financial, housing, and emergency assistance, as well as maintaining contact with those who are hospitalized.
  • Engage clients in skill-building activities related to daily living, budgeting, employment, and symptom management.
  • Work closely with the integrated health team to ensure seamless communication and coordination of care.
  • Document all service provision and maintain accurate records within required timeframes.

Required Skills

  • Strong communication skills to effectively engage with clients and collaborate with team members.
  • Knowledge of the Eight Dimensions of Wellness and experience providing holistic support.
  • Proficiency in documentation and administrative responsibilities.
  • Ability to build and sustain personal support systems for clients.

Qualifications

  • High School diploma or GED is required; an Associate's or Bachelor's degree in a human service-related field is preferred.
  • Relevant experience is required based on educational background, including work with individuals experiencing psychiatric, substance use, and developmental disabilities.
  • A valid driver's license and agency-mandated minimum automobile coverage are also necessary.
