Peak Health is seeking Health & Wellness Nurse Coaches for regions in Maryland, Virginia, Florida.
Regional details (5 different positions - all positions are Benefits-eligible):
1. Baltimore, MD: 32 hours/4 days per week (Includes Baltimore, MD, Annapolis, MD, Bowie, MD, Columbia, MD, Frederick, MD)
2. MD/VA: 40 hours/5 days per week (Includes Baltimore, MD, Hagerstown, MD, Elkridge, MD, Hunt Valley, MD, Frederick, MD, Columbia, MD, Myersville, MD, Winchester, VA)
3. Roanoke, VA: 24 hours/3 days (Includes surrounding area)
4. Richmond, VA: 40 hours/5 days (Includes surrounding area)
5. Southeast Florida: 32 hours/4 days (including Miami, FL, Ft. Lauderdale, FL, West Palm Beach, FL, Port Saint Lucie, FL, Plantation, FL, Boca Raton, FL)

ESSENTIAL DUTIES
Following is a summary of the essential functions for this job. Other duties may be performed, both major and minor, which are not mentioned below. Specific activities may change from time to time.
• Registered nurse travels to assigned client employment site to provide health evaluations for the participants, including:
Reviewing the individually completed Health History Questionnaire
Reviewing and interpreting individual blood lab test results
Obtaining individual weight, blood pressure, pulse, body fat or body mass, and fitness level.
Establishing individual goals by making recommendations for improving the individual's health risk.
• May conduct evaluation via teleconference when no site is available.
• May perform other tasks as assigned by Nurse Manager.
QUALIFICATIONS

Valid Registered Nursing License for state(s) in which they meet (call) program participants
Valid CPR certification
Valid Driver's License
Working vehicle and current personal auto insurance
Demonstrated proficiency in basic computer applications
Excellent motivational and coaching skills
The annual base salary for this position is $70,000 - $80,000. The starting hourly wage for this position is $33.65 - $38.46 (based on 40 hours.)
