What you’ll do in this role:

  • Meet with patients and families as scheduled by Journey, Palliative, and Hospice 
  • Conduct patient/caregiver visits to explain and discuss programs 
  • Implement appropriate patient follow up: phone calls, visits, etc. 
  • Receive and complete intake information for Hospice referrals 
  • Function as a knowledgeable resource person and mentor to Home Health, Hospice, and Palliative peers to educate on Residential offerings in relation to advanced chronic illness 
  • Maintain relationships with all existing referral sources 
  • Actively prospect for new referral sources based on the Agency’s scope of service 

We are looking for a compassionate Hospice and Palliative Liaison with:

  • High school diploma/GED 
  • Prior Home Healthcare/ Hospice or related work experience in the healthcare industry preferred 
  • Understanding of the Hospice philosophy and or Hospice experience preferred 
  • Empathetic, non-judgmental, tactful, responsible, and organized
