The Job
The Clinical Lead Nurse Practitioner will provide clinical and operational peer leadership to the Nurse Practitioners on the SCAN Mobile Care Team and provide primary care to SCAN members to achieve the Company’s mission to keep seniors healthy and independent.
You Will
Provide care to an assigned panel of members in their place of residence or virtually.
- Place of Residence: This includes conducting physical/wellness exams and assessments; conducting occasional urgent medical visits and telephonic triage to ensure timely, appropriate care and hospital avoidance if possible; administering vaccines; ordering and interpreting diagnostic tests; initiating goals of care conversations with members and their loved ones; and chronic disease management.
- Virtually: Initiate and complete virtual health visits with members as needed to promote accurate triaging and reduce drive time.
Support SCAN Mobile Care Team (SMCT) by:
- Assist with vacation/leave coverage of the SMCT nurse practitioners.
- Serve as a mentor and ongoing function as a direct resource for SMCT clinicians. Coordinate regular rounding with clinicians aimed at promoting clinical knowledge and skills development.
- Share findings from attendance at educational workshops and reviewing profession publications.
Provide Clinical Leadership as follows:
- Conduct first-hand Nurse Practitioner field observation to provide assistance and peer group feedback. Collaborate with leadership on how to best support NP performance based on performance observations and feedback.
- Participate in high level, collaborative interdisciplinary meetings aimed at identifying high-risk patients and developing effective interventions.
- Work closely with leadership in the design, implementation, and evaluation of evidence-based workflow processes. Review current workflows to identify inefficiencies and assist in creating solutions.
- Assist with team efforts to meet quality healthcare goals by closing care gaps in partnership with other care providers.
Pursue professional and technical knowledge by attending educational workshops and reviewing professional publications.
Participate in on-call program available to our members after hours and on weekends.
Participate with Integrated Care Sales team in events to build member awareness and educate communities.
Actively support the achievement of SCAN’s Vision and Goals.
This position is a patient-facing role.
Employees may be eligible for differential pay.
Your Qualifications
- Master’s degree with Nurse Practitioner Certificate, required.
- Education in Adult/Geriatric medical care, preferred.
- Current California Nurse Practitioner license in good standing.
- Wound Care Certificate and Palliative Care Certificate, preferred.
- 2+ years of related experience as a nurse practitioner, preferably with home-based visits, geriatrics, chronic disease management, and hospice and/or palliative care.
- Experience with virtual care/telehealth preferred.
- Knowledge of the treatment of complex chronic diseases, such as dementia, diabetes, heart failure, and COPD.
- Ability to identify team needs and coordinate with other resources/programs appropriately.
- Knowledge of HCC and ICD-10 coding and documentation.
- Knowledge of wound care and management.
- Knowledge of CMS guidelines and Medicare Advantage Managed Care.
- Comfortable with initiating goals of care and end of life discussions.
- Strong interpersonal skills and change agent mindset needed to build relationships with healthcare team and patients.
- Cultural competence and sensitivity required, multilingual skills preferred.
- Knowledge of and ability to maintain HIPAA requirements.
- Adaptable and open to change and problem-solving.
- Knowledge of and/or ability to learn basic technological skills (Word, Excel, PowerPoint, Outlook, EMR) and open to new technology.
- Travel 75+% of the time in the assigned area- Must have a valid driver’s license, automobile insurance and reliable transportation.
