THIS IS A 1099 20HRS PER WEEK CONTRACT ROLE!!!

JOB PURPOSE
Responsible to serve as a Qualified Health Professional (QHP) for our Virtual Primary Care, Urgent Care, Weight Management, Hospital to Home and RPM+ programs, responsible for conducting assessments and treatment planning for patients suffering from Chronic Health Conditions (CHCs). The QHP will evaluate patients for our remote tele-health programs involving the use of RPM (Remote Patient Monitoring) and other supportive measures including Clinical Health Coaching.

Here are our MUST HAVES:

A Master’s degree as a Nurse Practitioner/Physician Assistant, and at least three to five years of prior work experience in clinical setting.

Experience Specifically Required

  • Three years or more of Nurse Practitioner/Physician Assistant experience managing Chronic Health Conditions preferably in a primary care or specialist environment dealing with CHF, COPD, Diabetes, Obesity, etc.

CERTIFICATES, LICENSES, REGISTRATIONS, PROFESSIONAL DESIGNATIONS

  • A valid NP or PA license and must maintain their license for the defined geographic location of service (Nevada) required.
  • Multiple licensures in other states highly preferred

You will be responsible to:

  • Telehealth services include patient’s consultations through video conferencing (“Video Consultation”), electronic medical records, remote monitoring of vital signs, continuing medical education, and patient-focused wireless applications.
  • Consult with patients using the Practice’s telemedicine Platform through video conferencing or telecommunication.
  • Meet with an assigned patient via a scheduled, telehealth consultation to conduct a history and physical, which includes a review of medical and surgical record, medications, and allergies.
  • Develop a thorough list of the patient’s chronic conditions to establish the individual’s eligibility to participate in our Remote Patient Monitoring (RPM) and Clinical Health Coaching program.
  • Order the above services for those patients who are both eligible to receive services and willing to participate in the program.
  • Document the telehealth consultation upon completion of their assessment and ensures the proper CPT and ICD-10 codes are identified for medical billing by our internal staff.
  • Transition the patient to the assigned Enrollment Counselor and Clinical Health Coaching team to execute the RPM onboarding process.
  • Provide their availability to perform the consulting services (dates and times) not less than two weeks in advance of commencing the agreement.
  • Remain on-call an average of one day each week during normal business hours, typically from 8:00am to 5:00pm within their time zone.
  • Participate on bi-weekly update meetings with the National Medical Director or at other times as deemed necessary.
