Description

Diabetes & Nutrition Educator
 
Position Summary:
The Diabetes and Nutrition Educator (DNE) delivers comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of diabetes and cardiometabolic care.  The DNE is an integrative member of the clinical care team and works collaboratively with diverse patient populations to engage them in their health and well-being. The educator, through coordination with the care team, provides diabetes management and medical nutrition therapy to patients with attention to personalized treatment modalities to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes.

Position Qualifications/Essential Functions:
  • Provides comprehensive management of diabetes and associated conditions by integrating skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions, and self-management of diabetes into clinical practice.
  • Develops a plan of care that will optimize the patient’s control of Diabetes, improve quality of care and outcomes, and provide ongoing follow-up and communication.
  • Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient's assessed needs and goals and promotes successful self-management.
  • Applies the AADE7 self-care behaviors to educate on and initiate behavior change.
  • Follows the ADCES Diabetes Education Accreditation Program requirements which are based on the National Standard of Diabetes Self-Management and Support Guidelines. 
  • History of working with insulin management and comfortable with adjusting insulin under company policy/procedure. 
  • Familiar with diabetes technology and able to assist patients with using. 
  • Provides quality diabetes self-management education and medical nutrition therapy (limited to Registered Dietitians) in individual and group settings based on assessed needs. Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the person.
  • Identifies nutritional needs and develops nutrition prescriptions and interventions. 
  • Works with the care team and patient to review and monitor medications and treatment plans.
  • Effectively utilize electronic health record and verbal communication to concisely communicate relevant information to medical providers, and other treating providers in a timely manner.
  • Uses different outreach modalities to do esteeming patient education including in person, telephonic, portal communication and others.
  • Works as a member of the care team to support the patient in all aspects of care.
  • Strong proven ability to connect and deliver patient care in a virtual environment. 
  • Actively participates in the quality improvement (QI) process and adapts practice/process based on QI findings.
  • Applies business principles, systems practice, and population health management to support achievement of the Quadruple Aim (reduced costs, better outcomes, improved experience, and improved work life for healthcare providers).
  • Creates patient facing educational content (group classes/handouts) and educational content for providers being the source of expertise in cardiometabolic care.
  • Provides support in onboarding new care centers/providers/educators including presenting materials.
 
Additional Qualifications Preferred:
  • Ability to be creative and innovative in a fast-paced environment with a lot of ambiguity and constant change.
  • Clinical experience within an integrated care delivery system.
  • Flexible, high-energy level, team player, extroverted, self-starter, interest in larger systems issues in addition to individual patient care
  • Experience working in EMR systems required, Nextgen or Athena a plus.
  • Experience with Google Workspace.
  • Experience with functional nutrition concepts and nutraceuticals.
  • Experience with insulin pump therapy.
  • Comfortable with public speaking.
  • Bilingual or multilingual preferred.
 
Education/Experience:
  • Bachelor’s degree in related field required
  • Diabetes Educator Certification required
  • Registered Dietitian (RD) or Certified Nutritionist (i.e CCN, CNC, CNP, etc) – required
  • 3 years’ experience of working with diabetic population in a clinic setting required
  • Virtual care experience preferred. 
  • Masters degree in related field preferred
 
 
Compensation:  $32-$48/hr. & bonus eligible (national average, premium markets may vary)
