WHAT YOU’LL DO:
In close partnership with other Facktor team members, the Manager will support the scoping, implementation, and delivery of projects that are high-profile, technical, and grounded in best practice operational outcomes to clients in community healthcare. Projects will be seamlessly integrated into client operations and provide clients with operational, financial, strategic, and compliance. Additionally, the Manager will contribute to Facktor’s own product development, client relationship building, and business strategy as needed. This position combines operational analysis and project management with intense communication, engagement, and hands-on implementation with clients.
RESPONSIBILITIES:
- Work independently and collaboratively to provide high-quality strategic and functional guidance for Facktor clients.
- Build and maintain long-term relationships with clients as a trusted advisor.
- Collaborate across medical, behavioral health, dental, pharmacy, and social services to analyze existing workflows.
- Craft and implement streamlined programs to enhance efficiency and coordination across diverse healthcare services.
- Independently develop comprehensive documents, including policies, protocols, and procedural manuals.
- Implement innovative strategies and approaches to Enhance Care Management (ECM) practices, ensuring optimal patient outcomes and satisfaction through proactive interventions and personalized care plans.
- Create strategies to identify and address social drivers of health (SDOH) and enhance patient well-being
- Ensure all documentation and work products align with industry standards and regulatory requirements.
- Create and prepare project artifacts and deliverables for internal stakeholders, clients, and external compliance/regulatory bodies.
- Develop presentations using tools like MS PowerPoint, Prezi, or similar software, aligning with best practices for delivery by team members.
- Communicate with clients in an engaging manner, turning complex ideas into understandable tasks.
- Foster accountability within the project team through the accurate and timely creation and publication of meeting artifacts, proactive communication of project expectations, and the development of detailed project work plans.
- Facilitate communication between clients and project leads.
- Apply strong critical thinking skills to assess complex clinical operations challenges.
- Develop and implement innovative strategies to address operational inefficiencies and enhance overall performance.
- Identify root causes of issues and design interventions that seamlessly integrate with client operations for actionable recommendations.
- Foster collaboration between various healthcare disciplines to optimize patient care delivery.
- Investigate and research client issues through holistic investigations, utilizing document analysis, environmental research, interviews, and observations.
- Develop interventions fully integrated with client operations to ensure actionable and context-appropriate recommendations.
- Ensure consistency in approach related to analysis, techniques, and presentation of client recommendations/opportunities.
- Document recommendations in precise, thorough, and easily readable reports requiring minimal copy editing.
- Perform other duties as assigned.
WHAT WE LOOK FOR:
- Bachelor's degree in healthcare management, public health, business administration, or a related field.
- 5+ years of experience in their field + leadership.
- Subject matter expert in health center operations to analyze and craft workflows and programs.
- Experience leading the strategy and sustainability of Enhanced Care Management (ECM)
- Community Supports (CS) experience a plus
- Ability to see how their areas of expertise play a role in the bigger team.
- Moderate autonomy in managing projects, teams, or divisions.
- Leads and supervises a small team while providing professional guidance, makes decisions within their scope, and ensures goals are met.
- Networks, identifies potential clients, establishes and maintains client relationships, and creates pathways for senior leaders to identify upselling and cross-selling opportunities.
- People skills – warm and energetic personality who enjoys working independently as well as in a team environment.
- Proven experience in clinical operations management or consulting roles.
- Strong understanding of healthcare regulations, policies, and procedures.
- Excellent written and verbal communication skills.

- Project management experience a plus.
- Prior experience in a fast-paced work environment.
- Strong attention to detail in writing, analytics, and compliance.

LOCATION: Facktor supports a remote office environment. The Manager will be required to travel to client locations an estimated 20% of the time and as needed. Ideal candidates will reside on the east coast and have the ability to travel both locally and nationally.

SCHEDULE: Full-Time
SALARY RANGE: $110,000-$140,000
