About the Role

As a Behavioral Health Care Manager (BHCM) at Colla Health, you will deliver care to cancer patients through the Collaborative Care Model. Specifically, you will improve the health of your patients by providing brief behavioral health interventions and serving as the project manager of the collaborative care team, which includes the patient’s oncologist as well as a consulting psychiatrist. This is a remote opportunity with initial focus on virtual support to our partner practice in Reno, Nevada; and expanding to other areas as we grow.

You will be joining an early-stage company and will have the opportunity to impact the design and implementation of this innovative care model. 


What You’ll Do
  • Develop a trusted, effective working relationship with the partner oncology practice team. 
  • Work within a partner oncology group to support patients, clinicians, and practice staff with the identification and intake of patients needing behavioral health support.
  • Support the mental health of cancer patients on an assigned patient caseload.
  • Provide brief behavioral interventions tailored for the cancer setting. 
  • Screen and assess patients for common mental health and substance abuse disorders.
  • Facilitate patient engagement and follow-up care. 
  • Provide patient education about common mental health and substance abuse disorders and the available treatment options. 
  • Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications. 
  • Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment. 

  • About You
  • You hold one of the following licenses: Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW/LSW), Clinical Professional Counselor (CPC), Licensed Professional Counselor (LPC) Licensed Psychologist (PH.D or PsyD), or Certified Oncology Social Worker (OSW-C). 
  • Dedicated to providing evidenced-based care and have a demonstrated ability to collaborate and communicate effectively in a team setting. 
  • Prior experience interacting with physicians and collaborating with a multidisciplinary team. 
  • Have a passion for delivering quality behavioral health care. 
  • Ability to operate with a high degree of autonomy.
